The National Medication Errors Reporting Program, operated by the Institute for Safe Medication Practices, is a confidential voluntary program that provides expert analysis of system-based causes of medication errors. Report medication errors, preventable adverse drug reactions, close calls, or hazardous conditions as soon as they occur.

Partnership for Prescription Assistance
The Partnership for Prescription Assistance helps qualifying patients without prescription drug coverage get more than 2,500 medications through more than 475 patient assistance programs at no or minimal cost. Patients should call 1-888-4PPA-NOW (1-888-477-2669) to see if they are eligible.

The Script Your Future
campaign is designed to help patients with long-term health issues, including diabetes, asthma, high blood pressure or high cholesterol, take back their future by helping them take their medicine as directed
. This campaign provides tools to help patients manage medications and sample questions to help start a conversation with doctors, pharmacists and nurses about medication.
DEA National Prescription Drug Take-Back Day
Each year, the U.S. Drug Enforcement Administration schedules a National Prescription Drug Take-Back Day in April to help consumers safely dispose of unwanted or expired medications at over 5,000 take-back sites in all 50 states and U.S. territories. Locate a collection site near you
